Community health group (chg) is a locally-based nonprofit health plan serving members in san diego county. If you do not have a login, and need to submit a provider dispute, please contact provider services department at Member newsletter published by chg and tailored for members based on your plan.
Once we've learned that you've chosen chg for your health plan, we will mail you a member id card and other membership materials you'll need. Chg is required to provide timely access to care.
